Cartwright was raised on the outskirts of Dayton, Ohio, and won a school-wide talent contest when she was 10, scripting and performing a solo sketch based on Rudyard Kipling's "How the Camel Got His Hump". She worked in local theater beginning that summer, and her first paid gig was as a sidekick on a local radio show. She was mentored by Daws Butler, who voiced Huckleberry Hound and Yogi Bear. She began doing voice work in her early 20s, voicing Richie Rich's girlfriend and assorted background voices for Alvin & the Chipmunks. By 1987, Cartwright had worked on Pound Puppies, My Little Pony, and Popeye and Son, at which point she auditioned to perform Lisa Simpson's voice for The Tracey Ullman Show. Cartwright says she "couldn't wrap my wits around" the character of Lisa, but she was more intrigued by Bart. She asked to try out for that part, and Matt Groening said he knew she was right after hearing her read just one line.
